Consents
The Consents module lets you create and send consent documents to patients. Consent templates are reusable — build once, send to multiple patients. All signed consents are retained in the patient's clinical record.
Accessing consents
Navigate to Admin Portal → Consents to see your clinic's consent templates.
Consents are separate from intake forms. Forms are for collecting patient information (intake, health history). Consents are for obtaining patient agreement to treatment, procedures, and clinic policies. They each have their own admin portal section.
Creating a consent template
- Click New Consent Template
- Enter a template name and an optional description
- Write the consent body — this is the text the patient will read and sign
- Save the template
Templates are versioned. When you edit a saved template, the version number increments. Existing patient assignments retain the version of the template that was sent to them.
Sending a consent to a patient
From the consent template list:
- Click the Send button next to the template you want to send
- Search for the patient by name
- Click Send
The patient receives an email with a link to read and sign the consent. No login is required.
Tracking consent status
Click View All Assignments from the top of the consents list to see every consent sent across all templates.
The assignments table shows:
- Patient name
- Template name and version
- Date sent
- Signed status and date
Signed consent PDFs
When a patient signs a consent, Qliva generates a PDF record of the signed consent. The PDF includes:
- The consent text as it was at the time of sending
- The patient's name and the date signed
- The signature acknowledgement
Signed consent PDFs are accessible to treating practitioners in the clinical portal only. Clinic admin accounts do not have access to signed PDFs — this restriction protects patient confidentiality by limiting access to clinical staff.
Active / inactive templates
Templates can be toggled active or inactive from the template list. Inactive templates cannot be sent to new patients but existing assignments are unaffected.
