Qliva's consent system gives your clinic a simple way to send legally structured documents to patients and collect electronic signatures — without paper, printing, or scanning.
Consent Templates
A consent template is a reusable document that you draft once and send to patients as needed. Templates can be sent to any patient at any time from their patient record.
Creating a Template
Go to Admin Portal → Consents → New Template and fill in:
- Template name — an internal reference only; patients don't see this
- Description — a short explanation shown to the patient before they read the document
- Consent body — the full legal text the patient will read and sign
Activating and Deactivating
Only active templates can be sent to patients. Deactivate a template to remove it from the send workflow without deleting it.
Versioning
Each time you edit a consent template, Qliva creates a new version. Patients who have already signed a previous version are not affected — their signed document permanently captures the version that was active when it was sent to them.
This means you can update your consent wording at any time without invalidating prior signatures.
Sending a Consent
From any patient record, go to the Consents tab and click Send Consent. Choose the template to send and confirm.
The patient receives an email with a direct link to the consent document. No login is required — they can read and sign from the link alone.
Tracking
View all sent consents from Admin Portal → Consents → View All Assignments. The table shows:
- Which patient received which template
- When it was sent
- Whether it has been signed (and when)
Signed PDFs
Once a patient signs, Qliva generates a PDF that includes:
- The full consent text as it appeared at the time of sending
- The patient's name
- The date and time the consent was signed
- An electronic signature acknowledgement
