Qliva's form builder lets you create intake forms tailored to your clinic's workflow — from simple contact detail collection through to structured clinical assessments with mapped health data.
Building a Form
Go to Admin Portal → Forms → New Form.
Give your form a name and choose a classification:
- Administrative — general intake information (contact details, health history, preferences). Visible to admin staff.
- Clinical — health information, symptoms, medications, and other sensitive data. Visible to clinical practitioners only.
The classification affects who can see completed submissions, so choose carefully before sending the form to patients.

Any field except section headers, instructions, and signature can be marked as required. Required fields must be completed before the patient can submit.

Field Mapping (Clinical Forms)
Clinical forms can have individual fields mapped to structured data on the patient profile — such as medications, allergies, or health conditions.
When a patient submits a clinical form with mapped fields, those fields are queued for practitioner review rather than applied automatically. The practitioner opens the queue on the patient record and chooses to apply or dismiss each mapped value.
Reminder Schedule
You can attach a reminder schedule to any form that's linked to an appointment type. Qliva will automatically email the patient if they haven't completed the form before their appointment.
Available reminder intervals:
- 72 hours before
- 48 hours before
- 24 hours before
- 4 hours before
You choose which intervals to activate — for example, you might use only 72h and 4h. The reminder only fires if the form is still incomplete at that point.
Templates
Any form can be saved as a template for reuse. Browse and load templates from Admin Portal → Forms → Templates.
Templates are useful when you want to create variations of a standard form for different appointment types — for example, a base intake form with minor modifications for telehealth vs in-clinic visits.
Managing Forms
- Active / Inactive — inactive forms are hidden from the send workflow. Any forms already assigned to patients remain completable until submitted.
- Delete — forms are soft-deleted. They cannot be restored from the UI, but all completed submissions are preserved.
- Rename — click the form name to edit it inline.
